Несмотря на достижения современной медицины, излечение больных с опухолями глиального происхождения остается трудно достижимой задачей. Инвазивный характер и расположение в жизненно важных областях головного мозга делают этот тип рака сложным для хирургического вмешательства, а адъювантная терапия не приносит ожидаемых результатов. Частые рецидивы и злокачественность глиом обусловлены на; личием стволовых клеток, обладающих повышенной инвазивностью и устойчивостью к радио- и химиотерапии. Совершенствование технологий конструирования рекомбинантных вирусов позволило создать штаммы, проявляющие онколитическую активность в отношении глиальных опухолей. Большинство этих штаммов прошли первую стадию клинических испытаний и показали безопасность их использования. Несмотря на очевидную перспективность данного подхода, существующие штаммы недостаточно эффективны, что указывает на необходимость их совершенствования. В представленном обзоре рассмотрены наиболее успешные варианты онколитических вирусов, дошедших до клинических испытаний, а также обсуждаются перспективы новых подходов в виротерапии глиом.

ficiency of the existing strains is still far from being suffi cient for effectively curing the disease and require further im

provement. The r eview summarizes results obtained with the most successful variants of oncolytic viruses that come

down to the clinical trials and discusses the prospects for new approaches in virotherapy of malignant gliomas.

Stem cells that penetrated deeply into the brain tissue are the main reason behind the relapses of glioblastoma multiforme after surgery. Finding new approaches to counter such relapses, including those that make use of oncolytic viruses, is a pressing issue. This study aimed to determine the sensitivity of cells of human glioblastoma multiforme to non-pathogenic enteroviruses, in vitro and in vivo (mice xenografts model). Glioblastoma tumor cells were exposed to type 1 poliovirus (Sabin vaccine strain), Coxsackie virus A7 (strain LEV8), Coxsackie virus A9 (strain LEV9) and Coxsackie virus B5 (strain LEV14). The virus reproduction intensity and cytolytic activity were assessed through infection of monolayered glioblastoma cell cultures. The ability of glialoblastoma cell cultures (enriched with tumor stem cells) to build subcutaneous tumors in immunodeficient mice after those cultures were exposed to viruses signaled the effectiveness of glioblastoma stem cells destruction. The study revealed that Coxsackie virus A7 and type 1 poliovirus possess the most pronounced oncolytic and replicative properties when tested on gliblastoma cells infected with viruses in vitro and on subcutaneous tumor xenografts in immunodeficient mice (in vivo). Type 1 poliovirus and Coxsackie virus A7 virus prevented development of tumors when glioblastoma neurospheric cell cultures were preincubated with viruses before subcutaneous implantation. Coxsackie virus B5 only managed to reduce the number of tumors developed, and Coxsackie virus A9 did not affect the tumor development at all. Thus, a number of non-pathogenic enteroviruses strains can destroy glioblastoma's stem cells, i.e. they show promise in the context of development of therapeutic agents for relapse-free treatment of glioblastomas. © 2018 Pirogov Russian National Research Medical University. Что такое глиомы?

Глиомы – это группа злокачественных опухолей, развивающихся из нейроглий, которые формируются из вспомогательных клеток нервной системы. Глиомы являются самой распространенной и опасной формой рака мозга.

Наиболее агрессивной из глиом считается мультиформная глиобластома, медиана выживаемости которой составляет примерно 12-15 месяцев, и лишь 15 % пациентов доживает до трёх лет после постановки диагноза. До сих пор не существует каких-либо препаратов или методов лечения, которые могли бы существенно улучшить прогноз для пациентов с данным диагнозом. В свете этих мрачных данных выживаемости существует потребность в новых терапевтических стратегиях лечения глиобластомы.

Что такое виротерапия?

Онколитические вирусы становятся все более популярными для исследования в качестве противоопухолевых препаратов. Используя преимущества различных генетических дефектов опухолевых клеток, эти вирусы способны убивать раковые клетки, оставляя здоровые целыми и невредимыми. Кроме того, виротерапия стимулирует иммунную систему пациента на борьбу с онкологической опухолью.

Использование реовируса в лечении мультиформной глиобластомы

Учёные обратили внимание, что некоторые штаммы реовируса проявляют антиглиомную активность, то есть они специфически находят и связываются со злокачественными клетками глиомы, после чего уничтожают их. Это означает, что их можно использовать в лечении рецидивирующей злокачественной глиомы.

Что нас ждёт в будущем?

Реовирус является универсальным онколитическим вирусом, оказывающим влияние на широкий спектр злокачественных новообразований. Первые фазы клинических исследований при различных формах рака показали его безопасность и хорошую переносимость пациентами, что даёт надежду на положительный результат в проведении вторых фаз исследований, особенно в лечении злокачественных глиом.

В настоящее время проводятся многочисленные клинические исследования, находящиеся на второй фазе изучения, с использованием реовируса в сочетании с химиотерапевтическими средствами при злокачественных новообразованиях, таких как плоскоклеточный рак легкого, яичника, головы и шеи, поджелудочной железы и немелкоклеточного рака легкого. Кроме того, проводится вторая фаза исследования реовируса при различных метастатических опухолях, особенно при продвинутой меланоме и саркоме. Планируется также проведение второй фазы исследования внутричерепной инфузии реовируса у пациентов с мультиформной глиобластомой.

Литическая вирусная инфекция сопровождается образованием нового поколения вирионов и гибелью клетки. Опухолевые клетки обладают, как правило, повышенной чувствительностью к вирусам. Изучение явления вирусного онколиза указывает на возможность его использования в качестве альтернативного подхода к терапии рака. Способность вирусов селективно убивать клетки опухоли была отмечена давно, но лишь в последние годы, благодаря углублению знаний в области молекулярной биологии вирусов и клетки, а также разработке методов направленной модификации свойств вирусов, появилась возможность получения вариантов вирусов, обладающих максимальным терапевтическим потенциалом. Аденовирусы считаются одной из наиболее изученных моделей онколитических вирусов. Эти ДНК-содержащие вирусы удобны для генно-инженерных манипуляций и относительно малопатогенны. В представленном обзоре суммированы сведения о направлениях и подходах к созданию высокоэффективных онколитических аденовирусов, включая направленную генетическую модификацию, ускоренную селекцию онколитических вариантов после действия мутагенов, использование аденовирусов в качестве векторов для введения терапевтических генов, улучшение систем доставки, минимизацию действия иммунной системы организма и др. Состояние исследований в этой области позволяет надеяться на скорое введение многих вариантов онколитических аденовирусов в медицинскую практику.

. Активное исследование аденовирусов в качестве онколитиков началось с середины 1990-х гг., и количество работ, объектом которых является данное семейство вирусов, возрастает с каждым годом. Причина такой популярности аденовирусов обусловлена рядом факторов: они слабопатогенны для человека, способны заражать широкий круг клеток человека -как делящихся, так и не делящихся и относящихся к различным тканям, -обладают двухцепочечной геномной ДНК, что удобно для всевозможных генетических манипуляций; аденовирусы широко использовались в качестве модельного организма для изучения экспрессии генов, их геном неспособен интегрировать в геном клетки хозяина [1][2] [3] . .

Получен вариант аденовируса 2-го серотипа, названный Ad2del, имеющий делецию гена Е1В-55К. Показано, что благодаря внесенной модификации данный рекомбинантный штамм эффективно лизирует опухолевые клетки человека, такие как SW-480, С33А, DU-145, но в отличие от аденовируса 2-го серотипа дикого типа не способен активно размножаться в нормальных культурах клеток, таких как MCF-10А и MRC5. Проведенные исследования свидетельствуют о перспективности использования сконструированного рекомбинантного аденовируса Ad2del в качестве онколитического вирусного препарата.

The adenovirus serotype 2 variant with E1B-55K deletion (Ad2del) was constructed. It was shown that because of this modification the recombinant strain effectively lyse human cancer cells, such as SW-480, С33А, DU-145 but in contrast to wild type adenovirus serotype 2 it doesn’t replicate in normal cell cultures, such as MCF-10А and MRC5. This study showed the availability constructed recombinant adenovirus Ad2del as oncolytic virus agent.

In the fast growing area of vector systems, one of the most advanced is the replication deficient-recombinant adenovirus (Ad vector). It is remarkably efficient at transferring genes to a wide variety of target cells in vitro and in vivo and at ex­pressing exogenous genes at a high level [Crystal, 1995; Horwitz, 1996]. After in­fection, the Ad vector expresses the transgene in the nucleus of replicating and non-replicating cells in an epi-chromosomal fashion, where the duration of gene expres­sion is transient over time [Rosenfeld et al., 1991; Kass-Eissler et al., 1994; Smith et al., 1993; Engelhard et al., 1994; Yang et al., 1994a, b].

The human adenovirus E1B gene encodes a 55-kilodalton protein that inactivates the cellular tumor suppressor protein p53. Here it is shown that a mutant adenovirus that does not express this viral protein can replicate in and lyse p53-deficient human tumor cells but not cells with functional p53. Ectopic expression of the 55-kilodalton EIB protein in the latter cells rendered them sensitive to infection with the mutant virus. Injection of the mutant virus into p53-deficient human cervical carcinomas grown in nude mice caused a significant reduction in tumor size and caused complete regression of 60 percent of the tumors. These data raise the possibility that mutant adenoviruses can be used to treat certain human tumors.

Human interferon-beta (IFN-beta) has been widely used in gene therapy for its antitumor activity but its therapeutic effect is limited. The conditionally replicative adenovirus ONYX-015 (a E1B-55-kDa-deleted adenovirus) targets well to tumor cells, but is not potent enough to cause significant tumor regression. To solve these problems, a tumor-selective replicating adenovirus expressing IFN-beta was constructed in this study. The oncolytic adenoviruses were generated by homologous recombination in packaging cells. The expression of the IFN-beta protein was detected by enzyme-linked immunosorbent assay (ELISA). The antitumor efficacy of ZD55-IFN-beta was evaluated in cell lines and human hepatocellular carcinoma xenografts in nude mice. ZD55-IFN-beta can express much more IFN-beta than Ad-IFN-beta because of the replication of the ZD55 vector. Our data showed that ZD55-IFN-beta could exert a strong cytopathic effect on tumor cells (about 100-fold higher than Ad-IFN-beta or ONYX-015). Moreover, no obvious cytotoxic or apoptotic effects were detected in normal cells infected with ZD55-IFN-beta. The antitumor efficacy of IFN-beta could be significantly improved due to the increased gene expression level from the tumor-selective replicating vector. The oncolytic adenovirus expressing IFN-beta may provide a novel approach for cancer gene therapy.

To construct an adenovirus carrying SEA gene and regulated by telomerase reverse transcriptase (hTERT) and hypoxia-inducible factor (HIF) promoters and investigate its anti-tumor function in vitro, as well as its role in lymphocyte production. hTERT and HIF genes were cloned into adenovirus E1A and E1B shuttle plasmids. The control vector for SEA gene expression is under the regulation of CMV and SV40 promoters. Double regulation was obtained through homologous recombination. The positive clones of replicable adenovirus H2-SEA-Ad were selected by plaque assay. The adenovirus was purified, titrated, and DNA was verified by PCR. The obtained virus was used to infect EJ bladder tumor cells and the SEA Mrna, and protein expression was measured by RT-PCR, western blot, and immunofluorescence microscopy, respectively. Co-culture of lymphocytes and tumor cells was observed dynamically under microscope. The construction of shuttle plasmid p315-CSS-SEA was confirmed by PCR and DNA sequencing. Insertion of superantigen SEA gene in adenovirus (H2-SEA-Ad.SEA) was obtained by homologous recombination. In lymphocytes and tumor cell co-culture, the number of viable tumor cells in test groups was significantly lower than that in control group after 12, 24, and 48 h of treatment. Production of interleukin-2, interleukin-4, and tumor necrosis factor were higher in test groups than in control group. Expression of SEA gene in bladder tumor cells by adenoviral vector caused reduced tumor cell proliferation, as well as stimulation of inflammatory cytokine productions in co-cultures with lymphocytes.

The approaches include introduction of directed genetic modifications into the virus genome, accelerated selection of oncolytic virus variants following treatment with mutagens, the use of adenoviruses as vectors to introduce therapeutic gene products, optimization of viral delivery systems, minimization of the negative effects from the host immune system, etc. The dynamic development of studies in the field holds promise that many variants of oncolytic adenoviruses will find clinical application in the nearest future.

An overall view of oncolytic Ads, the merits and drawbacks of the various features of these vectors, and obstacles to further development and future directions for research. Ads are attractive for gene therapy because they are relatively innocuous, easy to produce in large quantities, genetically stable, and easy to manipulate. A variety of have been constructed and tested, in pre-clinical and clinical experiments. Oncolytic Ads proved to be remarkably safe; no dose-limiting toxicity was observed in any clinical trial, and the maximum tolerated dose was not reached. At present, the major challenge for researchers is to increase the efficacy of the vectors, and to incorporate oncolytic virotherapy into existing treatment protocols.
